If you can do the US Visa Waiver or have a US Visa, you may want to try AA.com to see if you can connect via Miami and book online with them. These prices do seem quite expensive. You can book from there in advance and maybe save a few $.
Unfortunately there are not a lot of flights out of SDQ that go to these places vs. connecting in Panama or the US.
I think Air Jamaica connects via Port Au Prince Haiti to Kingston. Why you'd risk likfe and limb going to Kingston I don't know, maybe you might try Montego Bay.
