According to Centers for Disease Control (USA) and equivalents from all over Europe, you SHOULD be concerned. At least concerned enough to consider malaria prophylaxis with Chloroquine. Ask your house doctor or check with CDC.
The outbreak of malaria in this region has had the health authorities surprised as the Dominican Government normally have the malaria situation under control. However, in just four weeks, at least seven Europeans have returned with malaria.
The risk of catching it is very small, and you can do a lot by preventing to get bitten by mosquitoes. When you stay in air conditioned resorts the risk is less still. BUT - all seven tourists that have been treated stayed at all inclusive resorts, according to the health authorities that have looked into the cases.
Hopefully, CDC and other health authorities will soon be able to change their recommendations again. But untill then I would take their advice. Rather safe than sorry. Malaria - although very rarely fatal - can be very unpleasant.
