Default Need help on translation please....

Could anyone please help me....

Regarding the translation of "Health Certificate for my pet."

I need to translate these three small paragraph below in Spanish.

The DR Consulate in Montreal (the only one in Canada), told me that I need to send the translate documents to them, I was going to bring it to DR, but they told me I cannot do this to have it legalize. :-(
So, now I'm stuck and need to send these documents by Wednesday of this week, so I can bring my dog with me.

If anyone can help me I would REALLY appreciate it.
