RD$30. They run about every 20-30 minutes at that time of night. You should get a seat as many get off at L'Union before the airport.

Take the "C" or Puerto Plata-Rio San Juan guagua to Puerto Plata RD$10
Take the Rio San Juan guagua to Sosua for RD$20 or Cabarete for RD$30.
