mickey...Miami beach has a sanitary system....as does a lot of the areas in Puerto Rico. They don't put in a septic tank that drains into a hole dug in the ground to sea level. Also they don't sit on a coral foundation that has 'channels' through it that the wave action and tide 'drains' into the sea.
It's all about geology and adequate sanitary filtration.
Also Steve G. there was an 'official' plan for the Juan Dolio area that was introduced about 25 years ago (I don't remember the exact year) that I saw personally before we bought our condo here in 1990.
Back in 1990 we we informed that the timeline for turning the beach road into a pedestrian walkway with access only for tourist buses and delivery vehicles and construction of a 'boardwalk' and sewage treatment plant was 5 years....so they're only 11 years late!
BTW Juan Dolio does have an aquaduct (al least old Juan Dolio), we're connected to it!
