You must have seen the 'challenges' driving on the roads here when you visited previously. My suggestion would be to rent a car with all the necessary cover plus extra eyes in the back and sides of your head and an insight into signalling (or not, as the case may be) and a crystal ball to really be able to know what is going on..... am absolutely certain that you will then have a great time! Enjoy your trip.
My only other comment and recommendation would be to use the new highway to go from Santo Domingo to Samana and then onto Santiago via Moca, if that fits in with your plans, as it will take a lot of stress out of the journeys.
