I know because I've done some extensive research on the airline, also using DOT and public records such as those you've cited. An airline only needs to file these documents to be current as of when the airline applied for service. It doesn't need to refile documents unless there is some major change in control, etc.
There will be an article in the September edition of "Airways" magazine that will explain the current situation/management at USA 3000. Angus Kinnear was recently brought into USA 3000 after he decided not to pursue the Canadian airline venture. I believe John Mullen has assumed the top role while Kinnear serves as COO.
