I was born in Puerto Plata How can I get a copy of my birth certificate. I have tried in the New York office but there is an error when they print it . My mother's name is misspelled and my father's name doesn't even appear on it. I have a copy of the my original birth certificate

I'm confused. If you were born in the DR why would NYC have it? Are you trying at the embassy or the Department of Health?

I was born in the Puerto Plata, but I'm an american citizen as per my father who is Puerto Rican. My mother is domincian. I live in NY

I'm was trying to get a copy at the Dominican consulate in NYC. the birth was recorded and registered in Puerto Plata

That won't happen. You need to get someone in POP to go to the Registry Office (probably at the Junta Central electoral) and with all of the information on you Original Birth Certificate, they can find your information and produce a new one for you.

They will need the DOB, where, who etc.
