extension of the tourist card

Hello,

how does the extension of the tourist card work? Can I extend the validity of the tourist card before it expires(by going to the immigration) or do I pay for the overstay when leaving the country?

If I understand this correctly, I have to pay just 5000 and I get my tourist card validity extended for 2 years? (source: Direccion General de Migracion)

Are there any special requirements to get this extension(like birth or police certificate)? Can I stay this way in DR indefinitely?

Have you heard of some cases in which the extension was denied?
