It's onomatopoeic, apparently, based on the sound of the horn. In Aymara, the indigenous language spoken in the Andean region, it means baby, for the same reason.

I think this word is original from Cuba; it was introduced in the DR by "Trespatines" in his radio show La Tremenda Corte, most popular comic’s radio show broadcasted in the DR for the past 70 years. Still broadcasted every in some of the Dominican radio station
KB- 94 FM Santiago, Every day at 1:30 pm.

the coach / bus - Spanish: el autobús - Dominican: la guagua (this term is also used in the Canary Islands (Spain), Cuba, Puerto Rico, but originating in the Canary Islands)

I am from Canary Islands and we have some words like guagua, papas etc. that you only listen here, in the rest of Spain they say autobus, patatas.
We don't pronounce the "Z" for example we write zapato but say sapato, we don't say vosotros, we say ustedes like in DR, for that when I go to DR they dont think that I am from Spain and when I go to Madrid they think that I am from Venezuela, Puerto Rico, DR,........

Andalucia too, carmeng - maybe not guagua but papas (as in papas a lo pobre), ustedes rather than vosotros to some extent and definitely the s sound for c and z. I've met people from the Canaries who sound a lot like Cubans - the roots of the Caribbean accent are definitely there.

In Mexico, camion and autobus are used in place of guagua.
